Dear Colleague

amicus /CPHVA Subscriptions Due

Accompanied with this letter is a statement for subscriptions due. These will now be sent out on a regular basis to members who are either in arrears or whose subscriptions are due.

Could you please forward the sum stated in the ‘amount enclosed’ box to ensure you that are covered until the calculated date.

If you have recently paid your subscriptions please ignore this bill.

To make things easier for you a direct debit form is attached which will ensure that your payments are made on a monthly basis and without you having to worry about it. Obviously it is important that you remain covered by professional indemnity insurance, as well as receiving the full range of our services. Paying by direct debit will ensure that you do not accidentally lose these by falling behind with your membership subscriptions

Yours sincerely

Jackie Carnell

Director

NOTE

CPHVA members will no longer receive an annual renewal for subscriptions due at the beginning of the year, instead they will receive a statement that is produced on a quarterly basis, which is in line with the rest of Amicus/MSF members. This statement will however only be produced if arrears are on a record or the annual subscription is due. If a member pays annually and has no arrears then the statement will only be produced annually, depending on the time of year that the subscriptions is normally paid.
